Managing Diabetes at Home

Diabetes occurs when our body is unable to produce enough insulin or does not use insulin effectively, resulting in high glucose levels. While diabetes itself is not life-threatening, it can lead to various health complications if not well-managed.

Here are some ways to effectively manage diabetes at home:

  • Take your insulin injections or oral medications as prescribed
  • Set regular meal times
  • Have a balanced diet with complex carbohydrates, lean proteins, and healthy fats
  • Identify and avoid foods that trigger glucose spikes
  • Monitor your blood sugar regularly
  • Stay active
  • Regulate stress levels
  • Communicate with loved ones about your diabetes management plan so they understand your needs and can support you

Caring for yourself or a loved one with diabetes while juggling work, family, and other responsibilities is not easy. Getting a support worker or nurse to create and execute a personalised diabetes care plan that incorporates healthier lifestyle changes can be helpful.

What we offer

Our nurses, caregivers, and therapists are trained and equipped to provide diabetes care and management at home, and can help with:

  • Vital signs monitoring
  • Meal preparations
  • Insulin injections
  • Medication reminders
  • Light exercises
  • Wound care
  • Post-surgery care
  • Assistance with activities of daily living (ADLs)

What to expect during a home-based diabetes care visit

During a diabetes care visit, a professional caregiver, nurse or therapist will come to your home to provide elder care services like assistance with daily living activities, nursing procedures, and therapy.

If you need nursing care, let us know the existing healthcare products available at home before the visit. If you do not have the appropriate products or tools required for the procedure, our Homage nurse will prepare the appropriate medical products required to carry out the nursing procedure

During the visit, the Homage Care Pro will introduce themselves, assess your health and preferences, and collaborate to create and personalise a suitable diabetes care plan. Insulin injections, medication reminders, meal preparation, wound care, and post-surgery care are common diabetes management services that we provide.

“At the start of visits, I greet my clients and check in on any past medical/social history. Knowing important things like previous falls or traumas can go a long way in ensuring safety and quality service. After every visit, I prepare notes which are service-related to assist other CPs who are handling the case after me.” — Care Pro Ralphael Lim, Registered Nurse

Wonderful app wonderful service

Have been using this for a year to help my grandfather who frequently gets aspiration pneumonia and has difficulty leaving the house. The app is so easy to use, and the cherry on top is the wonderful service behind the app. Had some difficulty finding a doctor recently and Hakim called me back personally twice to ensure a doctor was found. Thank you so much, cannot recommend this app more.

Doctor is qualified, friendly

Had a good experience with Homage's teleconsultation. The doctor is qualified, friendly and guided me on examination. She also explained my symptoms to me in detail, which is useful. The report provided has detailed information, and medicine was delivered to me on the same day even though my appointment was in the evening.
