Community Health Assist Scheme (CHAS)
The Community Health Assist Scheme (CHAS) allows all Singapore Citizens to enjoy subsidised rates for medical and/or dental care at participating General Practitioner (GP) and dental clinics. If you are a Merdeka Generation (MG) or Pioneer Generation (PG) cardholder, you will also be eligible for this scheme.
All CHAS, MG and PG cardholders will also receive subsidised referrals to public Specialist Outpatient Clinics (SOCs) and to the National Dental Centre Singapore and National University Centre for Oral Health Singapore*, if required.
If you are suffering from chronic conditions, CHAS complements the Chronic Disease Management Programme (CDMP), allowing you to use MediSave for outpatient treatments (for the same set of chronic conditions) covered under CHAS. Benefits
Benefits vary depending on the colour of your CHAS card. All Pioneer Generation (PG) and Merdeka Generation (MG) cardholders also receive subsidised rates at CHAS clinics.

Eligibility
All Singapore Citizens are eligible for CHAS. The benefits that you stand to enjoy will depend on your household monthly income per person or the Annual Value (AV) of the home, as outlined below:

If you already have the Public Assistance (PA) card, you are already eligible to receive full CHAS subsidies by showing your PA card.
You may calculate your household monthly income per person here.
^ Household monthly income per person is the total gross household monthly income divided by total number of family members living together. Gross monthly income refers to your basic employment income, trade/self-employed income, overtime pay, allowances, cash awards, commissions, and bonuses.
^^ The AV of your home is the estimated annual rent if it is rented out. It is assessed by IRAS.

How do I use my CHAS card?
Click here to view the full list of participating GPs and find a clinic near you. With over 1,000 CHAS clinics all over Singapore, it is easy to find one located near you. Look out for the CHAS sticker pasted outside of participating clinics as well.
Simply show your CHAS card to the receptionist and the receptionist will be able to guide and advise you.

How to apply
If you are a Singapore citizen aged 21 and above, you can apply with the following methods:
- Login to HealthHub with your Singpass here and submit an application
- Login to the CHAS online application system here to submit your application
- To submit a physical application, you can pick up a CHAS application form at any Community Centre (CC), Public Hospital, Polyclinic, or Community Development Council (CDC) or download the application form here. You can send the completed form to P.O. Box 680, Bukit Merah Central Post Office, Singapore 911536.
