What is a Severe Disability Assessment?
A Severe Disability Assessment evaluates a person’s ability to carry out Activities of Daily Living (ADLs) independently.
The 6 activities are bathing, dressing, feeding oneself, using the toilet, moving around, and transferring.
During the assessment, our assessors will seek to understand the care recipient’s medical history and request that they carry out a few tasks to assess their ability to perform these activities.
You may be asked to assist the care recipient during this process. A typical assessment takes between 45 minutes to 1 hour.
What is a Severe Disability?
An individual is considered severely disabled when they are unable to perform at least 3 of the 6 Activities of Daily Living (ADLs) independently.
When will my loved one need a Severe Disability Assessment?
Your loved one will need to undergo an assessment to make a claim or withdrawal under the following severe disability schemes:

How long will the Severe Disability Assessment take?
Depending on the care recipient’s condition, a typical assessment takes between 45 minutes to 1 hour.
What can I expect during the Severe Disability Assessment?
The assessor will first take a look at the medical history of the patient before carrying out the physical assessment.
If the patient is alert and communicative, the assessor will ask them to do a few tasks in order to evaluate their strength and ability for each activity of daily living.
If the patient has difficulty communicating, the assessor will have the primary caregiver assist with the physical assessment.
How can I help my loved one prepare for the assessment?
Before your loved one’s scheduled assessment, please prepare relevant documents such as:
- Your loved one’s NRIC
- Medical documents, such as a discharge summary, reports, or a doctor’s memo
- Completed claim form for supplements (applicable for claims under private insurers only)
- Relevant financial assistance scheme documents (if applicable)
- Periodic review letter (if applicable)
As a primary caregiver would know the care recipient’s needs best, it is recommended that their primary caregiver is present for the assessment.
What is the difference between an In-Person Care Assessment (IPCA) and a Severe Disability Assessment?
Homage’s In-Person Care Assessments (IPCA) provide a holistic assessment of your loved one’s living environment, as well as their cognitive and mobility skills. It is vital to help us match the right Care Pro to best meet your loved one’s care needs.
However, an IPCA will not allow you to make claims under a government or privately-administered severe disability scheme.
To qualify for withdrawals and claims under a severe disability scheme, your loved one must undergo a Severe Disability Assessment.
My loved one stays in a nursing home. Can they receive an assessment at their nursing home?
Yes. The assessment can take place at the care recipient’s place of residence, including nursing homes and residential care facilities.
When will I receive the results of the Severe Disability Assessment?
You will receive a letter and SMS from the Agency for Integrated Care (AIC) within 4 to 8 weeks.
How will the fee reimbursement take place if my loved one is assessed to be severely disabled?
The full assessment fee will be reimbursed by the government or your insurance company with the first payout or claim that you receive.
Am I eligible for the waiver of the assessment fee?
Your assessment fee will be waived if you are receiving assistance under:
- ComCare or public assistance,
- MediFund,
- Medical Fee Assistance Card (MFAC) or Medical Fee Exemption Card (MFEC), or
- Community Health Assist Scheme (CHAS) Blue.
If you fall into the above categories, you will not have to pay the assessment fees out-of-pocket.
Your fee will also be waived if you are receiving a periodic re-assessment.
What happens if my loved one is assessed to not be severely disabled?
You will not be reimbursed for the cost of the Severe Disability Assessment.
Is there a periodic re-assessment?
Yes. A periodic re-assessment may be necessary for a care recipient of a severe disability scheme to continue receiving payouts or claims.
You will be informed by AIC if a re-assessment is required.
