What is an In-Person Care Assessment?

The In-Person Care Assessment (IPCA) provides a holistic assessment of your loved one’s living environment, as well as their cognitive and mobility skills. It is conducted by one of our in-house Care Specialists. During the care assessment, areas that your loved one will be assessed include:

  • Mobility status
  • Communication ability
  • Cognitive function
  • Any special needs
  • Living environment

During the IPCA, our Care Specialist will interact with you and your loved one to better understand the current routine, discuss any care-related concerns and plan the most suitable care arrangements. They may also advise on suitable mobility aids and improvements to enhance the safety of the care environment.

An IPCA is an hour-long session which takes place at the stipulated care venue (generally in your loved one’s home). It is recommended that the primary caregiver is present, as they would be most aware of the exact care needs. Documentations such as medication should also be shared to provide a holistic understanding of the care needed.

Why Do You Need an In-Person Care Assessment?

The first step to the right care is a holistic understanding of your loved one’s care needs. Every individual is unique, and it is important for us to know more in-depth about the care needs you and your loved ones have.

An IPCA provides a more realistic understanding of your loved one’s care needs and their living environment. It also allows Homage to find the best Care Professional to be paired with your loved one through the creation of a detailed Care Plan.

When matching Care Professionals to your loved one, our technology-enabled process goes down to the minute details in your Care Plan, so that only Care Professionals with the relevant experience and competencies are selected.

When Do You Need An In-Person Care Assessment?

An IPCA is recommended for first time customers with Homage, or existing customers whose loved one’s medical conditions have changed.

What to Prepare When Going for an In-Person Care Assessment?

As the primary caregiver of your loved one, our Care Specialist will require certain information from you during the IPCA. This includes:

  • Your loved one’s pre-existing medical condition and history
  • Medications
  • Any documentations such as hospitalisation discharge summaries, medications etc.
  • Any food or drug allergies
  • Dietary preferences
  • Any pre-existing mobility issues

Please prepare these key information for the Care Specialist prior to the home visit.

How to Book an In-Person Care Assessment

To schedule an IPCA, download and log in to the Homage mobile app and follow the following steps:

  1. Tap the “Book” icon located at the bottom of the screen.
  2. Select the preferred type of healthcare category and individual this visit is intended for.
  3. Select Schedule Care Assessment and fill in the intended Care Recipient’s details and the location of care.
  4. Indicate your preferred date and time for the assessment and any special instructions.
  5. Select your preferred payment method from your E-wallet and confirm the appointment.

Congratulations, you have scheduled the In-Person Care Assessment successfully. Tap the “Activity” icon located at the bottom of the screen to view your scheduled assessment anytime.

Personalising Your Care Plan

Upon completion of the IPCA, our Care Specialist will summarise the information from the visit and develop a personalised Care Plan for you and your loved one.

In this Care Plan, it will detail your loved one’s:

  • Specific care needs (medical and/or daily living assistance)
  • Care routine
  • Living arrangements (any areas to pay particular attention to)
  • Medical history
  • Overall temperament, mood and hobbies
  • Ability to connect in terms of language

The Care Plan will be available in your Homage mobile app for your viewing. It can include photos and videos taken during IPCA to provide a better illustration of the care required. With these information, we will be able to match you with a Care Professional with the right skills set to care for your loved one.

How to schedule an In-Person Care Assessment?

The In-Person Care Assessment (IPCA) is conducted by our in-house Care Specialist with at least 30 years of nursing experience. IPCA is not required for Home Therapy, Home Medical and COVID-19 Swab Test services.

You can book a IPCA with us via our Homage mobile application.

Who are the Homage Care Professionals?

Homage Care Professionals are Singaporeans (and permanent residents) who are passionate and dedicated to delivering quality elderly care to families and care recipients. 100% local, they are trained caregivers from all walks of life.

Are there subsidies for Homage home care service?

Yes, you can apply for financial subsidy under the Home Caregiving Grant to defray the cost of home care. Learn more about the eligibility criteria and application process via the Agency for Integrated Care website.

Can 1 caregiver care for 2 care recipients at the same time?

To ensure you or your loved one receives undivided attention and care, each care recipient will be matched to a dedicated Care Professional who fits their needs and preference. If there are 2 individuals in the same home who needs care, a Care Professional would be present to care for each recipient respectively.

What are Homage home care packages?

We have interim to longer-term home care packages with varying care durations so that you and your loved one can find a care package which best fit your needs and preferences. Does Homage provide 24-hour home care?

Yes, we do provide 24-hour home care services. Two Homage Care Professionals will be scheduled for the visits, and care will be split into 12-hour shifts to support you and your loved one round-the-clock each day.

What is the shortest duration for a home care visit?

The duration of a Homage home care visit starts from just one hour, depending on the family’s preferences and your loved one’s needs.

Is home care and hospice care different?

Hospice care focuses on improving the quality of life and providing comfort to those with terminal illnesses, rather than focusing on a cure.

Home care is a broader term that includes hospice care and more. It covers assistance with activities of daily living such as showering, mobility support, companionship and staying active, enabling care recipients to stay safe and healthy in the comfort of home.
