1. Information We Collect and How We Use It

1.1.  We DKSH Singapore Pte Ltd (“DKSH”) or our third-party service provider acting on our behalf, will collect your personal information including, but not limited to, your name, caregiver name, contact information, gender, age, date of birth, and address.

1.2.  We will use your personal information, and sensitive personal information:
(a) for the DKSH Verzenio Sample Home Delivery Program (“Program”),
(b) to support you in managing your disease, including the home delivery Verzenio medication which has been prescribed to you by your physician
(c) to meet legal or regulatory obligations, inclusive of company record retention

2. Reasons We Share Your Information

2.1.  We may share your personal information, as well as sensitive personal information, with a third-party service provider to support you in managing your disease. The third parties’ responsibility is to home deliver your medication. They are not your diagnostic or treating physician. For all your medical questions, you must consult and contact your physician. All third parties that have access to your information have agreed to protect the information and to use it only as directed by us.

2.2.  We may also be required to disclose your information in response to lawful requests by public authorities, including to comply with national security or law enforcement requests.

3. How Long We Keep Your Information

Your Information will be saved for a period of time needed to fulfill legitimate and lawful business purposes in accordance with DKSH’s records retention policies and applicable laws and regulations.

4. How We Secure Your Information

We provide reasonable physical, electronic and procedural safeguards to protect information we work with and maintain. We limit access to your information to authorized employees, agents, contractors, vendors, subsidiaries, and business partners, or others who need such access to information to carry out their assigned roles and responsibilities on behalf of DKSH. Please be aware, although we try to protect the information we work with and maintain, no security system can prevent all potential security breaches.

5. Your Rights and Choices

5.1.  You have the right to request information from us on how your personal information is being used and with whom that information is being shared. You also have the right to request to see and get a copy of the personal information that we have about you, request its correction or request its erasure.

5.2.  There may be limitations on our ability to comply with your request.
