What Is Enhanced Home Personal Care (HPC+)?
Enhanced Home Personal Care (or HPC+) is Singapore’s new government-funded home care programme, introduced by the Ministry of Health (MOH) and administered by the Agency for Integrated Care (AIC).
It replaces the previous Home Personal Care (HPC) service with a more comprehensive, flexible, and better-coordinated approach to home-based care for seniors.

Government Subsidised Home Care Singapore: How Much Will You Pay?
HPC+ is subsidised by the government. The amount your family pays depends on your Monthly Per Capita Household Income (PCHI), calculated as your total gross household monthly income divided by the number of family members living together.

For households with no PCHI, Singapore Citizens may receive up to 80% subsidy for annual property values at or below $21,000.

What HPC+ Covers: 4 Care Modules, One Seamless Care Team
HPC+ is structured around four care modules. Your loved one can access one or more depending on their assessed needs, and modules can be added or removed as their condition changes.

Module A: Housekeeping
For seniors who need help keeping their home safe and clean
- Light and heavy housekeeping: sweeping, laundry, cleaning of fans, windows, and toilets
- Available when the client or caregiver is unable to manage these tasks independently
Module B: Personal Care
For seniors who need hands-on daily care and clinical support
- Assistance with bathing, personal hygiene, feeding, vital signs monitoring, medication assistance, cognitive stimulation, and simple exercises
- Higher care tasks supervised by a Registered Nurse: PEG tube care, simple wound dressing, urinary catheter care
- Extended hours support available in 4, 8, and 12-hour shifts (up to 360 hours per year) for hospital-to-home transitions or caregiver respite
Module C: Custodial Support
For seniors with complex needs and little or no caregiver support at home
- Daily check-ins functioning as a substitute caregiver
- Medication reminders, help managing bills and errands, transportation escort to nearby areas, and buying daily necessities
- Active hotline during office hours for clients to request assistance directly
Module D: Fall and Movement Monitoring
For seniors at risk of falls, especially those living alone
- Technology-enabled monitoring detecting extended inactivity or fall incidents
- Homage liaises with MOH-whitelisted technology vendors to ensure devices are always functioning
- 24/7 monitoring centre handles alerts, conducts triaging, and activates SCDF emergency response if required

Who Is Eligible for HPC+ in Singapore?
HPC+ is designed for seniors who need moderate to higher levels of home-based care. Eligibility is assessed through the InterRAI, a standardised care assessment tool administered by AIC.
Your loved one may be eligible if they:
- Are a Singapore Citizen or Permanent Resident
- Have care needs that can be supported at home
- Have been referred by a doctor, hospital discharge team, or medical social worker
Already receiving HPC services?
Existing HPC clients will be transitioned to HPC+ by end 2026. All clients currently on Modules A and B under HPC will be able to convert to HPC+ regardless of eligibility criteria. Homage will support your family through every step of this transition.

How to Access HPC+ Through Homage
1. Reach out to Homage or get a referral
HPC+ referrals come from a doctor, hospital discharge team, or medical social worker. If you are unsure where to start, fill out our self-assessment tool and contact Homage directly. Our care advisors will guide you through the process.
2. AIC conducts an eligibility assessment
AIC conducts an InterRAI Check Up (CU) assessment to determine your loved one’s eligibility and care needs. Homage receives the referral and assessment outcomes directly from AIC.
3. Care plan confirmed
Homage reviews the assessment and confirms which modules your loved one needs. For those with higher care needs, an additional InterRAI Home Care Suite assessment is conducted to further tailor the plan.
4. Care begins
Care services commence. Needs are reviewed every six months, or sooner if conditions change, with modules adjusted promptly to ensure the right level of support at all times.

What is the difference between HPC and HPC+?
HPC+ replaces the original Home Personal Care programme with a more comprehensive model. It introduces two new modules, Custodial Support (Module C) and Fall and Movement Monitoring (Module D), expands personal care to include higher care tasks supervised by Registered Nurses, and adopts a localised sub-region model for better coordination and faster response.
My loved one is currently on HPC. What happens next?
Existing HPC clients will be transitioned to HPC+ by the end of 2026, with a one-year grandfathering period from Q4 2025.
Your loved one continues receiving their current services during this period. All clients currently on HPC can convert to HPC+ prior to the end of 2026. Contact Homage now to plan the transition.
How are modules assigned?
Modules are assigned based on your loved one’s assessed care needs, determined through the InterRAI assessment conducted by AIC. Needs are reviewed every six months and modules are adjusted accordingly.
Does HPC+ operate on weekends and public holidays?
Yes. All HPC+ services operate daily from 8 am to 8 pm, including weekends and public holidays.
What if my loved one’s condition changes suddenly?
Homage can reassess needs and facilitate the steps to update the care plan promptly, adding modules or increasing care frequency as required.
Is HPC+ only for seniors who live alone?
No. HPC+ serves seniors across a range of living situations. Custodial Support and Fall Monitoring are particularly suited to seniors with low or no caregiver support, but Housekeeping and Personal Care are available regardless of living arrangement.
